The average Portuguese monthly wage will buy a year of private health insurance: the market's average premium is EUR 455.6 (USD 524) per insured person in 2025, while the public SNS that covers everyone charges nothing at all for a consultation1. That paradox - insurance cheaper than a phone plan, public care almost free - is the key to every other number on this page, from the EUR 39 doctor visit to the EUR 9,500 nose job.
So how much is health insurance in Portugal, and how much is everything around it? The insurance answer is EUR 456 a year on average, from EUR 10.57 a month at the floor, and the rest of the page prices the whole system: who pays for healthcare and how much, the SNS fees that survive (three emergency tiers), the dentist's ladder, botox, Ozempic, births, IVF, travel cover for visitors, a haircut, a tattoo, and what covid costs now2.
How much is health insurance in Portugal?
The measured answer: the average premium is EUR 455.6 (USD 524) per insured person a year, or about EUR 38 a month, on the Observatorio dos Seguros de Saude numbers for 2025 - the sector's reference statistics, up 9% from EUR 418.1 in 20241. Per policy rather than per person, the average is EUR 1,289, because most cover is family or group insurance sold through employers1.
The floor is lower than most visitors expect: Medis advertises hospitalisation-only plans from EUR 10.57 a month, which is why "how much is private health insurance in Portugal" has answers spanning a cinema ticket and a taxi ride2. Coverage is common but not universal: 3.87 million people carried private insurance at the end of 2025, 82,000 more than a year earlier, and 55% of them actually used it during the year1. How much is healthcare in Portugal, then? The insurance premium is only the entry ticket - the sections below add the public fees, the private consults and every price in between.

Who actually pays: the financing split
Portugal's health bill in 2024 came to EUR 2,765 per resident, and Eurostat's financing accounts show exactly who carried it: government and compulsory schemes paid 61.9%, households paid 28.7% out of their own pockets, and voluntary insurance covered 9.5%4. That out-of-pocket share is the number that surprises economists - it is nearly double the EU average of 15.2%, one of the highest in Western Europe4.
The trend line explains the anxiety. Per-capita spending collapsed to EUR 1,543 in the 2013 austerity trough, recovered through the 2010s, jumped with the pandemic, and hit EUR 2,765 in 2024 - 79% above the trough4. Our straight-line projection, an extrapolation and not a forecast, passes EUR 6,000 per person around 2050; the OECD's own 2025 profile, on a slightly different basis, already flags Portugal as spending about 20% below the EU average 5. As GDP's share, the bill has settled at about 10.2%, a touch under the pandemic peak of 11.2% in 20214.
- Government and compulsory schemes61.88%61.9%
- Household out-of-pocket28.67%28.7%
- Voluntary insurance9.45%9.4%
- Government schemes carry 61.9% of the bill.
- Households pay 28.7% out of pocket, nearly double the EU's 15.2%.
- Voluntary insurance is only 9.5% - smaller than most visitors assume.
Projection: linear trend of 2000–2024 yearly values, 95% prediction range. Our calculation, not a forecast.
- Spending fell to EUR 1,543 per person in the 2013 austerity trough, then climbed 79%.
- The 2024 figure of EUR 2,765 is the highest in the series.
- The straight-line extension passes EUR 6,000 around 2050.
The public SNS: what is free and what is not
The Servico Nacional de Saude that covers every resident used to charge small "taxas moderadoras" at every turn; the 2022 reform abolished them for consultations, so a GP visit or a scheduled hospital appointment now costs nothing at all6. What survives is a single fee list: walk into a hospital emergency room without calling SNS 24 or bringing a referral, and leave without being admitted, and you pay EUR 18 at a polyvalent ER, EUR 16 at a medical-surgical one, EUR 14 at a basic one6.
Everything else on the SNS menu is free or exempt: referred emergencies, admitted patients, pregnant women, minors, donors and low-income households all skip the fees entirely6. This is why the private-insurance question is smaller in Portugal than in the United States: the public system already answers the cheap end of the market, and the EUR 14-18 ER fee - about USD 16-21 - is the only number most residents will ever see3.
- The most expensive SNS fee in the country is USD 20.70.
- Referred or admitted patients pay none of these fees.
| Health-centre (GP) consultation | free |
| Scheduled hospital consultation | free |
| ER with SNS 24 / health-centre referral | free |
| ER admitted to hospital | free |
| ER without referral, no admission (polyvalent) | EUR 18 (USD 20.70) |
| ER without referral, no admission (basic) | EUR 14 (USD 16.10) |
| Exemptions | pregnant women, minors, low income, donors, and more |
How much is private healthcare in Portugal?
The private answer starts with the consultation: private GP visits run from EUR 39 at independent clinics to about EUR 60, booking platforms charge EUR 44 for any specialty, home visits cost EUR 55, and top private-hospital specialists reach EUR 1507. In USD that is a EUR 45-173 spread - real money, but a fraction of a US specialist bill.
Above the consultation sit the procedures, and this page prices the ones people actually ask about: dental work from EUR 33 to EUR 5,000, botox from EUR 180, rhinoplasty from EUR 5,500, an IVF cycle at EUR 5,250, a private birth at EUR 3,800-7,0007. How much does it cost to see a doctor in Portugal, finally, depends on one choice: EUR 0 with an SNS appointment, EUR 39-150 walking into a private clinic7. The chart shows the private ladder at its four most common rungs.
- A GP visit without insurance starts near USD 45.
- Even the priciest specialist consult tops out at USD 172.50.
| GP or travel medicine, independent clinic | 39 | 44.85 |
| Any specialty, booking platform | 44 | 50.6 |
| Doctor at home | 55 | 63.25 |
| Typical private range | 50 | 57.5 |
| Top specialist, private hospital | 150 | 172.5 |
The dentist: the ladder from EUR 33 to EUR 5,000
Dentistry is where Portugal runs most private, and the documented 2026 ladder starts surprisingly cheap: EUR 33 buys a consultation that already includes one basic treatment such as a cleaning or a simple filling7. From there each rung multiplies: a ceramic crown costs EUR 350-756, an implant with its crown EUR 800-2,500, fixed braces EUR 2,000-3,500 for the full 24-month treatment, and Invisalign EUR 1,500-5,000 depending on how many aligner stages the case needs7.
How much do braces cost in Portugal, then? Budget EUR 2,000-3,500; how much does a crown cost in Portugal? EUR 350-756; how much do dental implants cost in Portugal? EUR 800-2,500 per tooth; and how much is invisalign in Portugal? EUR 1,500-5,000 - the premium option that now prices above wire braces at most clinics7. Searches for OECD Portugal dentist consultations per capita usually want the context rather than the price: Eurostat's healthcare-activities statistics count dentist visits per inhabitant across the EU, and Portugal's dental market is overwhelmingly private, which is exactly why the clinic ladder above - not an SNS tariff - is the price list that matters4.
- Invisalign tops the ladder at about USD 3,700 for a full treatment.
- A crown costs roughly a tenth of an implant-and-crown package.
- The EUR 33 consult already includes one basic treatment.
| Consult (includes one basic treatment) | 33 | 37.95 |
| Ceramic crown | 350-756 | 517.5 |
| Implant with crown | 800-2500 | 1,893.75 |
| Fixed braces, full treatment | 2000-3500 | 3,162.5 |
| Invisalign, full treatment | 1500-5000 | 3,737.5 |
Cosmetic: botox and the nose job
Aesthetic medicine publishes its prices openly, and the clinic tables agree within a few euros. Botox starts at EUR 180-200 for a single zone - the frown or the crow's feet - EUR 270 for the whole upper face, and up to EUR 450 for face and neck together; bruxism treatment runs EUR 270-3707. How much is botox in Portugal, in practice? About EUR 190 a zone, a third of typical US pricing7.
Surgery is the big-ticket tier. How much is a nose job in Portugal? Rhinoplasty averages EUR 5,500-9,500 including surgeon, anaesthesia and theatre, with Lisbon and Porto clinics at the top of that band7. The USD equivalents - about USD 218 a zone and USD 6,300-10,900 for the nose - explain the growing medical-tourism flow into Portuguese aesthetic clinics, which price at half of American counterparts while using the same products3.
- A single botox zone costs about USD 218 in Portuguese clinics.
- Rhinoplasty runs USD 6,300-10,900, with Lisbon and Porto at the top.
| Botox, one zone | 180-200 | 207-230 |
| Botox, upper third | 270 | 310.50 |
| Botox, face and neck | 450 | 517.50 |
| Rhinoplasty (nose job) | 5500-9500 | 6325-10925 |
Ozempic, Wegovy, Mounjaro: the pens and their prices
The answer to how much is Ozempic in Portugal splits into two economies. In the reimbursed one - type-2 diabetes, prescribed under SNS rules - the pen costs about EUR 10-12 at the pharmacy, and the state spent EUR 22.8 million in 2024 reimbursing 1.6 million packages8. In the private one, the same pen sells through online clinics at EUR 240 and up, Wegovy launched at EUR 244.80 per injection, and Mounjaro's top dose reaches EUR 3378.
Per month of treatment, that is USD 276-388 for the private pens against USD 13.80 for the reimbursed one - a 20-fold gap that explains the waiting-list politics around these drugs3. For scale, the same pens list at EUR 887 and EUR 1,164 per pack in the United States, so Portugal's private prices sit well below American list prices while staying far above the reimbursed EUR 128. Obesity indications are private-pay everywhere in Portugal; only diabetes qualifies for the comparticipacao8.
- Private-pay pens cost USD 276-388 a month in Portugal.
- Reimbursed Ozempic for diabetes costs about EUR 12 a month.
- The state reimbursed EUR 22.8 million of pens in 2024.
| Ozempic (semaglutide) | Reimbursed, type-2 diabetes | 12 | 13.8 |
| Ozempic (semaglutide) | Private, online clinic | 240 | 276 |
| Wegovy (semaglutide) | Private, launch price | 244.8 | 281.52 |
| Mounjaro (tirzepatide) | Private, top dose | 337 | 387.55 |
How much does it cost to give birth in Portugal?
How much does it cost to give birth in Portugal? In the public SNS, nothing: childbirth carries no moderadora, the midwife and obstetrician are covered, and the baby's first checks come with the room6. The private route, chosen with insurance or without, has documented prices: a normal birth runs EUR 3,800-4,200 without cover, and a caesarean EUR 6,000-7,0008. Civil servants' ADSE reimburses private hospitals EUR 1,965 for a natural birth and EUR 3,005 for a caesarean, which shows what the state itself judges these episodes to cost 8.
The maternity bar chart puts the three numbers side by side: USD 0 for the SNS route, about USD 4,600 for the private normal birth, USD 7,450 for the private caesarean. Prenatal care in the SNS is free throughout, and the private alternatives price individually per visit at the consultation rates in the section above8.
- A private caesarean runs about USD 7,450 all-in.
- The SNS charges nothing for a birth or for public IVF cycles.
IVF: three free tries and the private price
Portugal's public system is unusually generous here: each couple - or woman without a partner - is entitled to three artificial inseminations and three FIV/ICSI cycles free in public centres, within clinical eligibility rules set by the national council CNPMA9. How much does IVF cost in Portugal, then, is a question of patience versus speed: the public route costs time on a waiting list, the private route costs money now.
The private benchmark is EUR 5,250 per cycle at IERA Lisboa, one of the country's established fertility clinics, with medication on top7. That is about USD 6,000 per attempt at current exchange rates - comparable to southern-Europe peers and roughly a third of US pricing3. Fertility preservation and donor programmes price separately; the table keeps the public entitlement and the private cycle side by side.
| Childbirth | free, no moderadora | 3800-7000 | 4370-8050 |
| IVF / ICSI | 3 cycles per couple (CNPMA) | 5250 per cycle | 6037.50 |
| Abortion (to 10 weeks, Law 16/2007) | free | 500-750 | 575-862.50 |
Abortion: legal, free in the SNS, priced in the private
Abortion is legal on request to ten weeks of pregnancy under Law 16/2007, and the SNS performs it free, including the required pre-procedure consultation 9. How much does an abortion cost in Portugal, then, is a question only private clinics answer with a price list: medication from EUR 500, surgical procedures EUR 650-750 depending on anaesthesia and weeks of gestation 7. Those are among the lowest documented prices in Western Europe, and no referral or residence requirement applies - the law asks only that the woman request it in writing after the statutory reflection period9.
Travel insurance: the Schengen floor
Visitors face a simpler market than residents. A Schengen visa requires travel insurance with at least EUR 30,000 of emergency medical cover valid across the whole Schengen area for the whole stay, and how much is travel insurance to Portugal at that floor? Plans start at about EUR 2 a day - USD 2.30 - from the major aggregators3. Without a visa the cover is not mandatory, but the SNS bills non-EU visitors directly for hospital care, so the insurance question is about risk, not rules6.
EU visitors sit in between: their European Health Insurance Card covers SNS treatment at home-country conditions, which for most episodes means the free consultations and the EUR 14-18 ER fee above6. The practical advice the numbers support: buy the EUR 2-a-day policy for the visa or the peace of mind, and treat the SNS as the fallback rather than the plan.
| Minimum medical cover for a Schengen visa | EUR 30,000 (about USD 34,500) |
| Validity | whole Schengen area, whole stay |
| Typical cost | from about EUR 2 (USD 2.30) per day |
| Without a visa | not mandatory, but ER bills without it are yours to pay |
Personal care: tretinoin, haircut, massage, tattoo
The pharmacy starts the ladder: how much is tretinoin in Portugal? The classic Ketrel cream costs EUR 6-10 and its generics under EUR 3, all prescription-only 7. The salon follows: how much is a haircut in Portugal? EUR 13.75-18.75 for men and EUR 15-27.50 for women at ordinary neighbourhood salons, with promo chains undercutting at EUR 6-87.
Wellness and body art complete the list. How much is a massage in Portugal? EUR 30 buys a half-hour back session at therapy clinics and EUR 40-60 the standard hour7. How much is a tattoo in Portugal? Studios charge about EUR 40 an hour, small pieces run EUR 80-160, and the platform average for a finished tattoo is EUR 3007. In USD the whole personal-care menu spans USD 3.45 to USD 345 - cheaper than the same menu in Paris or New York by roughly the same third that shapes every other price on this page3.
- A small tattoo costs about USD 138; studios charge near USD 46 an hour.
- The pharmacy tretinoin tube is the cheapest item on the page.
| Tretinoin cream (prescription) | 3-10 | 7.5 |
| Haircut, man | 13.75-18.75 | 18.7 |
| Haircut, woman | 15-27.50 | 24.4 |
| Massage, 30 minutes | 30 | 34.5 |
| Massage, 60 minutes | 40-60 | 57.5 |
| Tattoo, small piece | 80-160 | 138 |
| Tattoo, hourly rate | about 40/hour | 46 |
Covid in 2026: tests, vaccines and the regional archive
The covid questions in search bars now point at a historical record, so the page answers them with the archive's own numbers. Portugal confirmed 5,671,684 cases and 29,663 deaths, a fatality rate of 0.52%, with the first case recorded in Porto on 2 March 202010. How much of Portugal is vaccinated? 8,909,769 people - 86% of the population - completed vaccination, briefly the highest share in the world10.
What it costs is the other half of the question. How much is a covid test in Portugal today? Routine testing has ended and labs no longer publish covid tariffs; the 2021 benchmarks were EUR 40 for the state-paid PCR, EUR 25 privately and EUR 20 for an antigen test, and a symptomatic patient today simply pays the ordinary consultation prices in the sections above8. And Portugal covid cases by region, the query that used to refresh daily? The DGS regional dashboards stopped with the emergency in 2023; the archive remains online, and no new regional counts have been published since10.
- The state paid EUR 40 per PCR under its 2021 agreement with labs.
- Private PCR tests costed EUR 25 and antigen tests EUR 20.
| Total confirmed cases | 5,671,684 (DGS archive) |
| Total deaths | 29,663; fatality rate 0.52% |
| Cases by region today | no longer tracked; the DGS regional dashboards ended with the emergency and the archive stays online |
| Fully vaccinated | 8,909,769 people = 86% of the population, briefly the world's highest share |
| Covid test today | routine testing ended; the 2021 benchmarks were EUR 40 state / 25 private PCR, 20 antigen |
How good is Portuguese healthcare?
Prices are only half the value question. In Numbeo's user survey - the health care index by country 2026, Numbeo Portugal included - Portugal scores 72.0, level with the United Kingdom's 72.4 and behind only a handful of systems headed by Czechia at 75.611. The objective map tells a similar story of capacity: Eurostat counts 688 practising doctors per 100,000 people in the Lisbon region and 561 in the Norte, against 322 in the Alentejo - which is why the map below puts the darkest districts where the specialist care, and the private prices of this page, concentrate4.
The honest caveat sits with the out-of-pocket share from the start of the page: 28.7% of all health spending comes straight from Portuguese households, nearly double the EU average, so the system that is cheap for visitors asks real contributions from its own residents4. Cheap at the pharmacy window, cheap at the SNS door, and financed by taxes and co-payments everywhere else - that is the trade the numbers describe.
- The Lisbon region concentrates 688 doctors per 100,000 people, the country's densest medical market.
- The Alentejo has barely half of Lisbon's density, at 322 per 100,000.
- Prices follow the doctors: specialist care clusters where the map is darkest.
Conclusions
- Private health insurance costs EUR 455.6 (USD 524) per person a year on average, from EUR 10.57 a month at the floor, and 3.87 million residents carry it1.
- The public SNS charges nothing for consultations and only EUR 14-18 for a direct ER visit without referral - the reason insurance stays optional rather than essential6.
- Portugal spends EUR 2,765 per resident on health (2024), 79% above its 2013 trough, but households still fund 28.7% out of pocket, nearly double the EU average4.
- The private price ladder runs from EUR 33 dental consults to EUR 9,500 rhinoplasties, with the weight-loss pens at EUR 245-337 a month privately against EUR 12 reimbursed7.
- Reproduction is the standout: births, IVF cycles and abortion are free in the SNS, while the private route costs EUR 3,800-7,000, EUR 5,250 and EUR 500-750 respectively9.
- The system's quality scores match its price: 72.0 on Numbeo's 2026 index, 86% fully vaccinated during covid, and 688 doctors per 100,000 in Lisbon against 322 inland11.
Methodology
- Health expenditure and financing: Eurostat SHA series, 2010-2025
- Regional capacity: Eurostat practising doctors by NUTS2, 2020
- Insurance: Observatorio dos Seguros de Saude 2024-2025 (via ECO)

- Covid: DGS archive totals, vaccination and 2021 test-price benchmarks
Definition. 'Health insurance' means voluntary private medical insurance (seguro de saude), distinct from the public SNS that covers all residents. The average premium is the market-wide mean reported by the Observatorio dos Seguros de Saude; individual premiums vary with age, cover and group discounts. 'Out-of-pocket' is Eurostat's HF3 scheme share. Market prices for consultations, dental, cosmetic and personal care are documented clinic price-list ranges - no authority sets them. USD converts EUR at the ECB reference rate of 1.15 (mid-September 2026).
Data sources. Eurostat hlth_sha11_hp, hlth_sha11_hf, hlth_rs_prsrg (API, official); Observatorio dos Seguros de Saude via ECO (Apr 2026); ERS and SNS fee summaries; CNPMA and Law 16/2007 via ERS; OECD Country Health Profile 2025; DGS covid archive via Wikipedia infobox; Numbeo Health Care Index 2026 (labelled); clinic and platform price lists (KeepWells, LON Clinic, Dental Light, Fada do Dente, Clinica Dentaria de Lisboa, Minimal Medical, Living Clinic, IERA Lisboa, Castrelos, Clinica dos Arcos, Sanjam, Vitor & Vitor, Fixando), all labelled industry; media price reports (euronews, CNN Portugal, SIC, Expresso, Publico, ECO).
Calculations. USD conversions at 1.15 (work/calc.py). Chart mid-points are the mid-point of each documented range. The spending projection is a least-squares line on the Eurostat 2010-2024 series extended to 2050 - our calculation, not a forecast; the OECD profile's EUR 3,001 (2023) uses a slightly different expenditure basis and is kept out of the chart. Doctor-density map values are NUTS2 regions shown on their principal districts (labelled on the map).
Limitations. No official tariffs exist for private consultations, dental, cosmetic or personal care: all are clinic price lists and platform averages, labelled industry, quoted as ranges. Insurance premiums have only two open annual anchors (2024, 2025), so the premium carries no trend chart. Ozempic's official pharmacy PVP was not extracted from Infarmed's database; reimbursement and online-private figures are used, both sourced. Covid test prices are 2021 benchmarks because labs stopped publishing covid tariffs; regional covid counts stopped in 2023 with the DGS dashboards.
